A clear referral supports connected care.
Practical information for GPs and other treating professionals.
For GPs & referrers
Support a clear referral
Check the current intake notice before sending a referral. Referral review considers clinical needs, urgency, age, location and suitability for telehealth; a referral does not guarantee an appointment.
- Include the reason for referral, relevant clinical history, current medicines and relevant risk information.
- Confirm patient contact details, location and consent to share information. Identify any guardian or authorised contact arrangements.
- Use only a clinic-confirmed secure channel for the referral letter and supporting documents.
Uncertain whether a referral was received?
Do not assume a form or upload was received unless the receiving service confirms it. Verify receipt through an established clinic contact before resending. For urgent needs, use an appropriate urgent-care pathway rather than waiting for a website response.
View intake notice ↑Before sending a referral
Check the intake notice at the top of this page. This website does not accept referrals, uploads or booking requests.
Confirm a secure destination
Use only a clinic-confirmed secure channel. Do not send health records to an unconfirmed email address. No public referral destination is available on this website.
Consent and communication
Confirm consent to share the referral and supporting information. For a child or adolescent, identify the parent, guardian or authorised contact arrangements relevant to the referral.
After referral review
Clinical fit, urgency, age, location and telehealth suitability inform review. An appointment may be offered only where the service is suitable and has capacity. Correspondence with the referrer may follow a consultation where appropriate and authorised.
